Best LIS Software for Small Clinical Labs

Vendor documents read

For lab directors, lab managers and physician office lab managers choosing a laboratory information system for a small CLIA lab with a waived and moderate-complexity menu. We read six vendors' own product pages, help-centre articles, brochures and interface specifications, and 42 CFR 493 on eCFR, and checked each vendor against nine duties the rule puts on the lab.

The verdict by use case

Best overall for a small nonwaived lab

CGM LABDAQ

The only vendor whose own pages name all nine CLIA rows here: its option pages document a proficiency testing export and a records export for inspectors, and its brochure names QC rules that hold results.

Best for physician office labs

CrelioHealth

A browser-only system whose help centre shows critical-report sign-off limited to permitted users and QC values reviewed before they reach the Levey-Jennings chart.

Best for a lab adding pathology or billing

LigoLab

Clinical and anatomic pathology plus revenue cycle on one platform, with audit-trail functionality its own tier page pitches as a deterrent to unauthorized alterations.

What CLIA asks of an LIS, row by row

The plate map scores vendors against duties the lab carries under 42 CFR 493, not against feature lists. CLIA certifies laboratories, never software, so no LIS is “CLIA certified”. The question is whether the system makes each duty cheap to prove to a CLIA surveyor. Each row below gives the rule in its own words and one question to put to the vendor in a live demo.

The CLIA rule behind each plate-map row and the demo question that tests it
RowWhat 42 CFR 493 saysAsk in the demo
AQC and patient test records “for at least 2 years”; a copy of every final, preliminary and corrected report “at least 2 years after the date of reporting”; pathology test reports “for at least 10 years”.§ 493.1105Open a final report and its corrected version from two years ago, on screen, now.
BTest records must show “the identity of the personnel who performed the test(s)”; after an error the lab must “Maintain duplicates of the original report, as well as the corrected report.” CLIA has no section titled audit trail: an LIS trail is how most labs meet both.§ 493.1283, § 493.1291(k)Change a released result. Does the original stay readable, with who changed it and when?
CControl results must be acceptable “before reporting patient test results”, and the lab “must document all control procedures performed.”§ 493.1256(f), (g)Will the system hold a patient result when today's QC failed or was never run?
DPatient name and ID number (or a unique identifier), “The name and address of the laboratory location where the test was performed”, the report date, the test, the specimen source when appropriate, the result with units, and the condition of any unacceptable specimen.§ 493.1291(c)Does every test line carry the performing lab's name and address, send-outs included?
EThe lab must “immediately alert” the person who ordered the test when a result shows panic or alert values.§ 493.1291(g)Where does the system record who was called, by whom, and at what time?
FPT samples are tested “in the same manner as it tests patient specimens”, and PT records are kept “for a minimum of two years from the date of the proficiency testing event.” Choosing a PT program: CAP vs API proficiency testing.§ 493.801(b)Can a PT sample be accessioned without creating a claim, and its results sent to the PT program?
GThe lab must “ensure confidentiality of patient information throughout all phases of the total testing process”, and results go only to authorized persons.§ 493.1231, § 493.1291(f)Can a medical assistant enter results without being able to release them?
HResults sent to interfaced systems must arrive “accurately and reliably”. Which results go to the health department, and in what format, is set by each state, not by CLIA.§ 493.1291(a); state lawWhich state health department feeds do you run today, and who maintains them when the state changes format?
IRecords must be “accessible and retrievable within a reasonable time frame during the course of the inspection.” After a survey with citations, CMS's manual gives the lab 10 calendar days from receipt of the CMS-2567 to return a plan of correction.§ 493.1773(c)Pull every QC failure on one analyzer for last quarter while the surveyor waits.
Note

A proposed change to report content

CMS's proposed rule CMS-3478-P, published 8 October 2026, would require the CLIA number and the laboratory's name “as it appears on the CLIA certificate” on every test report (a revised § 493.1291(c)(2)). It is a proposal, not a current rule; comments are due 7 December 2026. Ask each vendor how fast a report-template change reaches your lab if the rule is finalised.

Physician office labs: when an LIS earns its keep

Most physician office labs never need one. CMS's September 2026 count shows how the office labs split by certificate, and the split decides the software question before any demo does.

114,313

Physician office labs on CMS's register, CLIA-exempt states excluded

CMS, September 2026

84,829

Of them hold a Certificate of Waiver

CMS, September 2026

16,123

Hold a certificate for provider-performed microscopy

CMS, September 2026

13,361

Hold a certificate of compliance (9,104) or accreditation (4,257): the nonwaived office labs

CMS, September 2026

A waiver lab must “Follow manufacturers' instructions for performing the test” (§ 493.15(e)(1)). The quality-system rules in Subpart K, where most of the plate map's rows live, apply to labs that perform nonwaived testing (§ 493.1200(a)), and waiver labs are not subject to biennial inspections. For an office runningurine dipsticks, urine pregnancy tests and fecal occult blood, results charted in the EHR and a log of whatever QC the kit inserts call for usually do the job.

The calculation changes the day the office adds one moderate-complexity test. CMS says the certificate “must be for the most complex category of testing the laboratory performs”, and from then on the office carries QC records, proficiency testing, report content and an inspection every two years. That is the point where the thirteen thousand or so nonwaived office labs start shopping, and three things matter more for them than for an independent lab.

1. The EHR interface

Orders should leave the EHR and results return to it without retyping, because a retyped result is a transcription error waiting for § 493.1291(a). All six vendors here advertise EHR interfaces; ask for a live site that runs on your EHR by name, and who pays when the EHR vendor upgrades.

2. A mixed waived and moderate menu

Office labs keep their waived kits beside the new analyzer. The LIS should record the waived results too, so one patient chart shows one history, while QC lockouts apply only where the rule asks for them.

3. A clean billing hand-off

Most office labs bill through the practice's own system, so the LIS has to pass a correct charge, not run the claim. Under a waiver or PPM certificate Medicare pays for tests subject to CLIA edits only when the claim carries the right HCPCS code with the QW modifier, per CMS's October 2026 quarterly update. CGM documents medical necessity checking as a LABDAQ option; ask every vendor how its charge file maps to your practice management system. Billing systems themselves are compared in lab billing software compared.

Takeaway

Waived only: stay in the EHR and keep the manufacturer's instructions on file. Adding a moderate-complexity test: shortlist on the EHR interface first, then run the nine questions above in the demo.

When a vendor calls it a LIMS

The vendors blur the words themselves: Clinisys markets one platform as either, and CrelioHealth's own help centre calls its product “Crelio LIMS”. Judge by the output, not the label: if the system prints a report a physician acts on and sends a charge to a payer, it is doing an LIS's job under CLIA, and every row in the plate map applies to it. The full difference is in what an LIS is, and how it differs from a LIMS.

How LIS quotes are built

No vendor here publishes a full price list, and this page carries none. What the vendors do publish is how they charge, and it points at the lines to read in any quote. The same reading applies to every system in our lab software comparisons.

  • Interfaces. Dendi prices its subscription by the number and type of integrations; MEDFAR says it absorbs the upfront integration cost for office labs. Every analyzer and every EHR is a line.
  • Hosting. CGM offers LABDAQ on site or as subscription cloud hosting; CrelioHealth and Dendi describe themselves as built cloud-native.
  • Options. CGM sells the proficiency testing export, Data Miner and inventory as options. Price the rows you need, not the base system.
  • Size of lab. LigoLab's demo form asks for your expected monthly software budget before it books a call.
Caution

The line most quotes leave out: leaving

A copy of each report has to stay retrievable “at least 2 years after the date of reporting” (§ 493.1105), and that clock keeps running after you switch systems. Put read access or a full export of old reports in the contract before you sign, not when you leave.

Sunquest, Fletcher-Flora and other names you will meet

Sunquest. Clinisys says “Sunquest is now Clinisys” and that Sunquest's products carry on inside its laboratory solution. The LIS sold today as Clinisys SQ Lab is pitched at multi-disciplinary labs managing complex workflows, so it was not scored here; ask Clinisys whether it suits a single small site. Orchard Software went the same way: its products are now branded Clinisys, and Clinisys says they stay available and supported. We put the two names small labs ask about most side by side in CGM LABDAQ vs Orchard Harvest.

Fletcher-Flora. Its FFlex eSuite LIS was on the early shortlist, but no Fletcher-Flora product site answered when we checked on 22 October 2026, so there was nothing first-hand to score. Ask the vendor for current documentation if it is on your list.

CGM SCHUYLAB. CompuGroup Medical's second LIS, listed beside LABDAQ in CGM's own menus for physician office and reference labs. If you are already talking to CGM, ask which of the two it would install and why.

Note

How this comparison was made

We read each vendor's product pages, help-centre articles, brochures and interface specifications, 42 CFR 493 on eCFR, CMS's CLIA laboratory statistics and the State Operations Manual, Chapter 6, last on .

A filled well means we verified the claim against a document we could read and link: the vendor's own documentation or a regulator's record. An open ring means the vendor says so in its own material, but we found no document to check it against. A blank well means we found nothing either way, which does not prove the product lacks it.

Verdicts are editorial: no vendor can buy a verdict position or displace another vendor. This page carries no prices and no ratings; ask each vendor for a quote and a demo on your own test menu.

On this page a filled well needs a vendor document whose job is to describe the function: a help-centre article, an interface specification or certification record, or a module or option page. A claim on a general product page or in a brochure counts as stated. The LABDAQ brochure CGM links today carries a 2019 file date, so confirm its rows in the demo.

The six vendors were chosen because each sells an LIS to small CLIA labs or physician office labs in the US today. Vendors that could not be reached first-hand are named in “Other names you will meet” and not scored.

Questions labs ask

Does CLIA require a lab to use an LIS?

No. § 493.1291(a) asks for “an adequate manual or electronic system(s)” to get results to the report accurately, and § 493.1283 asks for “an information or record system”. Paper is legal. An LIS earns its keep by making each duty quick to prove when a surveyor asks.

Is any LIS “CLIA certified”?

No. CLIA certificates are issued to laboratories, and § 493.5 lists the five a lab can hold. A vendor that says “CLIA-compliant” means its software supports duties the lab carries. Ask which duty, and to see it in the demo.

Our office lab only runs waived tests. Do we need an LIS?

Usually not. A waiver lab must follow the manufacturer's instructions (§ 493.15(e)(1)) and is not subject to biennial inspections, and the Subpart K quality-system rules apply to nonwaived testing. Results charted in the EHR normally do the job until the office adds a moderate-complexity test.

How long must the LIS keep results?

QC and patient test records for at least 2 years, and a copy of every final, preliminary and corrected report at least 2 years after the date of reporting (§ 493.1105). Pathology test reports: at least 10 years. If you also do immunohematology, the blood-bank record rules in 21 CFR 606.160 apply on top.

Can we switch LIS and leave the old data behind?

Not inside the retention window. The reports must stay retrievable for 2 years after each was issued, whichever system holds them. The usual routes are read-only access to the old system, or importing its reports as documents.

What would CMS's October 2026 proposed rule change for an LIS?

Mainly the report template. Proposed rule CMS-3478-P would put the CLIA number and the laboratory name as it appears on the CLIA certificate on every test report. It is a proposal, not a current rule; comments are due 7 December 2026.

Is Sunquest LIS still sold?

Under another name. Clinisys says “Sunquest is now Clinisys”, and its current LIS is sold as Clinisys SQ Lab. See other names you will meet.

Why does no vendor get a score or a price?

Because the plate map measures something you can check: whether a vendor document shows the function a CLIA duty needs. Scores hide that, and prices depend on your interfaces and menu. Ask each vendor for a quote on your own test menu.
